How to Register

If you are needing assistance this holiday season please pick up a registration form which are available starting November 1st at the following locations;

Sylvan Lake Community Partners
Sylvan Lake Library
Sylvan Lake Family and Community Centre
Sylvan Lake Nexsource Centre

Registration Day

When: Sunday, November 19, 2023

Location: 90B Hewlett Park Landing

All applications for the 2023 season will require an appointment to register.

Must provide the following information to register:

Completed registration form
Government issued photo ID for all adults
AHC# Cards for all adults and children
Proof of residency (two of the following; gas or electricity bill, Town of Sylvan Lake bill, cell phone bill, cable/internet bill, or rental agreement)
Proof of household income (last two paystubs, information for Child Tax Benefit, Social Assistance, EI, and/or Pension, or bank statement showing all deposits for the last month)

Registration is open from November 19th to December 15th. If you have missed our registration day, please call us at (403)348-9993 to make other arrangements for your form. Please note that late registrations (after Registration day) cannot be guaranteed specific gifts. Help our elves by registering as soon as possible.
